Dear Defiance College Student-Athlete:

The Defiance College Athletic Communications Department publishes informational material on its student-athletes and keeps in contact with the media in their hometowns. To ensure we have information that is current and accurate, please take a few minutes to thoroughly complete this form. By filling out the form, you give Defiance College permission to use any information provided, as well as any statistics and information during your athletic and academic career at DC, for publicity purposes, and to the extent the Family Education Rights and Privacy Act (FERPA) applies to this information, you waive any right not to have this information released.
